What will I learn on this module?
This module explores the physiotherapy role in the management of long-term conditions with a particular emphasis on supporting self-management. The module aims to equip you with the knowledge and skills needed to help people to take greater control over their health. An increasingly large proportion of the population are living with at least one long term condition. We will explore the application of behaviour change theory to those living with long term conditions through supporting them to develop the skills and knowledge that they need to manage their condition in the most effective way possible.
How will I learn on this module?
You will learn through lectures, practical session, seminars, tutorials and independent learning. The module staff will support your learning and development across the module. There will be a strong emphasis on professional skills, including communication, and the knowledge that underpins this, with opportunities to evaluate your learning formatively through active engagement in feedback.
Lectures will introduce you to key ideas and theories around the role of the physiotherapist in long term condition management, and seminars will develop your understanding of approaches used to support service users through their lifetime. Practical sessions will enable you to develop appropriate intervention skills as well as enhancing your communication and educational skills.
You will be expected to:
• Attend teaching sessions
• Be dressed appropriately for practical sessions
• Behave in a professional manner
• Engage and contribute to the learning and development of yourself and others
How will I be supported academically on this module?
Module information, assessment requirements and expectations, and important module announcements will be made through the eLP. Material to support your learning will be uploaded in advance of sessions. You will be provided with content verbally and guided through tasks during timetabled sessions. A dedicated assessment session will afford you the opportunity to ask assessment related questions in class. The Module Leader is there to support you with any queries that you may have about the module content. You will have the opportunity to receive formative feedback on your development during the module, and you will receive feedback on summative work to help you identify areas of strength and areas for development. There will be the opportunity to discuss your feedback with a member of staff. There is also further academic skills support available from the University Library services.
What will I be expected to read on this module?
All modules at Northumbria include a range of reading materials that students are expected to engage with. Online reading lists (provided after enrolment) give you access to your reading material for your modules. The Library works in partnership with your module tutors to ensure you have access to the material that you need.
What will I be expected to achieve?
Knowledge & Understanding:
1. Discuss the principles underpinning the management of long-term conditions and demonstrate understanding of how this relates to the role of the Physiotherapist
2. Demonstrate understanding of the appropriate use of self-management strategies for individual service users and their carers and how these relate to theories of behavioural change and health promotion
Intellectual / Professional skills & abilities:
1. Justify the selection of appropriate approaches to assessment and a range of physiotherapeutic skills that are considerate of an individual’s facilitators and barriers to effectively self-manage their condition
2. Demonstrate the ability to use therapeutic communication and educational interventions appropriately to enable people to take more control over their own health
Personal Values Attributes (Global / Cultural awareness, Ethics, Curiosity) (PVA):
1. Demonstrate insight into the impact that living with a long-term condition might have on an individual and those who care for them, and reflect this in the ability to communicate empathetically
How will I be assessed?
The module will provide you with a number of opportunities for formative assessment of a range of practical and communication skills. For example, you will get verbal feedback from lecturers in practical and seminar activities. There will also be opportunities during seminars for you to receive feedback from peers.
Summative assessment will be via a 2500 word written essay, providing an evidence-based critique of a long-term condition management strategy.
Individual written feedback will be provided with the opportunity to discuss with a module tutor. General cohort feedback will be provided via the e-learning platform.

Module abstract
In this module you will develop an understanding of how Physiotherapists can bring together both educational strategies and therapeutic skills to help the patient to achieve supported self-management of their condition. As well as gaining an understanding of the broad principles underpinning the management of long-term conditions, by the end of the module you will have had the opportunity to discuss the Physiotherapist’s role in the management of some commonly encountered long term conditions. The theme of person-centred care will be central to this. The content of this module will complement the Physical activity and health module as there are many transferable skills between health promotion and supporting people to live well with a long-term condition, however this module will help you to consider in more detail some of the barriers that we may need to help those living with a long-term condition overcome.
